Actually, he'll have the surgery after he comes back and has two or three more terrible outings.  That's the Nationals Way!

cortisone shot is to relieve the pain

I'm no medical expert, but it seems weird that they wouldn't remove everything with surgery :|

Cortisone is to relieve swelling, and if it's a nerve issue, it can makes things far more painful.

Cortisone does more than just block pain.  It also promotes healing with certain types of injuries.
