Most teams don't.   The normal shift alignment gets absolutely mangled if the runner goes.  You're left with the first baseman parked near first, the 2B standing out in RF, and two gigantic holes.  At that point, basically any ground ball not pulled hard is a hit, and a likely 1st-to-3rd.
