Serious question:    Can he only be in the game as a non-pinch hitter when Cincy comes to bat or did Dusty change the lineup during that half inning?    How would that work?

C&D said he wasn't a pinch hitter the second time because he wasn't replacing anybody, he was already in the game.

The decision was made when the inning was over. When the double switch is made while a team is on defense then the slots have to be decided on the spot.

They are joking because at that point Scherzer was still the pitcher of record and Drew (who Sanchez replaced defensively) still had an AB.

Yes. There could be a situation where the pinch hitter hits multiple times in an inning and the inning results in a walkoff and therefore never entered the game defensively (and even in a non walkoff if he doesn't enter the game defensively it still counts as 2 official PH AB's).
